2016 Food Vendors Info & Application “Sold Out”

The Middletown Historical Society invites new and returning food vendors to participate in this year’s Olde Tyme Peach Festival.  We will have 2 types of vendors:


  1. Non-profit clubs, churches, youth groups and service organizations, which will be located in the Forest Church parking lot. Cost: flat fee of $50.00 per 10’x10’ space.
  2. Commercial and other vendors  will be located in the Town Hall and Middletown Hardware parking lot.  Cost will pay $175 for a 10×10 foot space.

All vendors must provide their own tables, tents, chairs and displays.  All food vendors must have a day permit issued by the Food Services Section, Division of Environmental Health.  Click on the Health Permit below, download and fill out the application permit, then email it back to me at middletownpeachfestival@gmail.com.  I will submit all the application together 30 days before the event. All Food vendor’s will receive the permits the day of the festival.  Inspectors will be present at the festival to inspect all Food vendor’s.  Setup time will start at 6:00a.m. until 8:00a.m.

Please note, THIS IS A RAIN OR SHINE EVENT. Submit your form below and then make your payment through PayPal. Payments must be received to reserve your booth.
